Fees and Payments
We accept cash, credit card (Mastercard/Visa) and check. If applicable, co-payments are due at the time of service at check-in.
For self-pay patients, we offer a 15 percent discount for treatments paid for in their entirety by cash, check or credit card
on the day of service.
Appointments and Cancellations
We respect your time and strive to offer appointment times that fit into today's busy schedules. We provide evening hours,
and same day appointments. We estimate the time needed for your visit based on what you tell us, so please tell our staff about
all of your medical problems you will need to discuss with your provider.
We understand that sometimes you need to change your appointment time. We ask that you notify us 24 hours prior to your scheduled
appointment if you need to cancel for any reason. This allows us to accomodate another patient if needed. Please call ahead if you will
be late. If you arrive late, please understand you may be asked to reschedule or you may need to wait for a "work in" spot.
Insurance and Insurance Cards
Your health insurance card is just like a credit card. Please bring it with you to every appointment for any needed verification.
Please familiarize yourself with the rules and covered benefits of your insurance policy, we do not know what is covered under
your individual policy. Ultimately it is your responsibility to follow the rules of your insurance plan to ensure proper payment. Some
insurance plans require a specific laboratory or hospital system for diagnostic tests so you must show us your insurance card at each

Identification
To protect your health information there may be times when we will ask for a picture ID. Please make sure that you are able to present
this when asked. If you are interested in obtaining a username and password to access the
patient portal we will make a copy of your
picture ID.
Prescription Refill
We expect you as the patient to contact our office if you need a refill. We will no longer take calls from pharmacies. (more detail)
Prescriptions will likely not be refilled if your provider has not seen you within the past 12 months. Please allow 48-72 hours for all
refills.
Referrals
Most insurance plans do not require authorization for a referral, but we ask that you please check with your insurance carrier prior
to requesting a referral.
After Hours Call
If you become ill or have an emergency after
hours, we ask that you call 911 or go to the nearest
urgent care facility or nearest emergency room.
