Health Insurance

We make every effort to obtain accurate information from your insurance company. However, the information we receive from your insurance company is not a binding agreement for payment.

It is important that we have a copy of your current medical insurance card. Please bring your card to EVERY appointment for any needed verification. It is very important that you inform us as soon as you know when there is a change in you coverage.

Please familiarize yourself with the rules and covered benefits of your insurance policy, we do not know what is covered under
your individual policy.  Ultimately it is your responsibility to follow the rules of your insurance plan to ensure proper payment. Some
insurance plans require a specific laboratory or hospital system for diagnostic tests so you must show us your insurance card at each
visit.
