Cardiac Rehab

The Cardiac Rehabilitation Program at Holm Medical Clinic (Located at Fitness Forum Sports & Wellness) works to answer patients’ questions and guide and assist them in adjusting to lifestyle changes following a cardiac event. With the Cardiac Rehab Nurse's guidance, patients are able to identify their specific risk factors for cardiac disease and outline goals and plans to alter them. Our staff helps patients return to a healthy and productive life.

Cardiac Rehabilitation, or "rehab" is a medically supervised program that combines progressive exercise with education and counseling.

Your Health Care Provider may recommend cardiac rehab to help you recover and improve your health after you have had: a heart attack, bypass surgery, stent placement, valve repair, valve replacement or heart transplant. Cardiac Rehab is also recommended for people with other heart conditions including, but not limited to: Angina, heart failure & peripheral arterial disease.
