Please familiarize yourself with the rules and covered benefits of your insurance policy, we do not know what is covered under
your individual policy. Ultimately it is your responsibility to follow the rules of your insurance plan to ensure proper payment. Some
insurance plans require a specific laboratory or hospital system for diagnostic tests so you must show us your insurance card at each
visit.
You will be asked to pay your co-payment at check in. If you do not have your co-pay you may be asked to reschedule your appointment.
